2025-2026 Chicago Hawks Uniform Order
All players must fill out the uniform pack order form below. Season fees will cover the cost of the Uniform Kit (home and away game socks, pant shells, and a warm up jacket and pants.) When the uniform order is placed, you will have the option to purchase additional items, such as extra game socks. Game Jerseys are not included in the uniform pack and will be purchased for an additional fee ($145.80 for one, $280.80 for the set).
Midget teams will be given a winter coat and two pairs of home and two pairs of away socks as part of their uniform kit. Each player will be given one coat during their time in the midget program. New Midget players need to email Equipment Manager, Nick Skweres for a coupon code for their winter coat. If a new coat is needed it can be purchased separately.
Practice Jerseys and practice socks can be reused this season. If you are new to the Club, your practice jersey and socks will be provided to you. Any returning players that need a new jersey or practice socks will be required to purchase one. If any items need to be replaced throughout the season they will need to be purchased.
All first year Hawks and those new to the organization must purchase a set of home and away jerseys. Returning players are not required to purchase new jerseys as long as the jerseys still fit, are in great condition and the player intends on keeping the same number from the previous season(s).
Refunds will only be provided for jersey purchases of players who have not been OFFERED a roster spot within the Chicago Hawks Hockey Club. All jersey number conflicts will continue to be resolved in favor of the longest-tenured player with the most consecutive years in the Club. In the event two or more players with the same number are placed on the same team, the player(s) with less longevity will be required to change their number. Depending on the numbered jerseys the Club has in stock, a player may incur the cost of an additional set of jerseys.
All new players to the Club, including first-year mites, will be assigned a number based on availability and numbers already in use. Uniform numbers will be assigned based on the previous years numbers, new number requests may not be honored and are assigned at the discretion of the Equipment Manager.
Any uniform questions prior to being placed on a team can be directed to the equipment manager, Nick Skweres, at equipmentmanager@chicagohawks.com. After you are placed on a team you will refer all questions to your team manager.
Baseline Concussion Testing
It is MANDATORY for all Chicago Hawks/Darien Phantoms athletes to have a Baseline Concussion Test completed prior to the start of the season. NEW THIS YEAR, Once tryouts and team selection is completed, all Chicago Hawks/Darien Phantoms families will receive a unique link for baseline concussion testing. All athletes will take the test at home on their own personal device. Results will be stored and used as a reference point should an injury occur. Your child will NOT be allowed to compete until the test is completed.
What is ImPACT Neurocognitive Baseline Testing?
Using the ImPACT Neurocognitive computerized software, your son/daughter will complete a series of "brain games" used to measure their level of symptoms, reaction time, verbal and visual memory, and processing speed.
Baseline Testing includes:
- A computerized Baseline ImPACT test used to measure the level of symptoms, reaction time, verbal and visual memory, and processing speed.
- If an injury does occur, results will be used to help athletic trainers and/or medical professionals with a more accurate diagnosis.
- Athletes may be recommended to perform Physical Therapy and further testing based on severity of injury.
